I Marie HOUDE, about to be liberated this date under-Ticket-of-Leave hereby agree to proceed to the home of my Sister-in-law, Madame Fred Gauthier who has assumed full responsibility for my care and treatment in the future, which may be necessitated by my present condition. I willingly accept my release under this additional condition and relieve any official of the Penitentiary or Department of any responsibility should my physical condition become aggravated or any expense incurred thereby.
